- Responsible for advocating for and with our members to ensure informed decision making, informed consent, and appropriate guardianship. - Responsible for scheduling, leading and actively collaborating with our members and their interdisciplinary team to conduct meetings and assessments ensuring the development of a comprehensive, person-centered Life Plan that reflects the person’s needs and desired life goals. - Implement, update, and monitor Life Plans and facilitate individualized Life Plan reviews and approval processes. - Ensure integration of all needed and preferred supports and services (i.e., medical, behavioral, social, habilitation, dental, psychosocial, and community-based, and facility-based long-term supports and services, etc.). - Maintain ongoing contact with the critical people in a member’s life. - Ensure timely submission of all documentation (Life Plan, Progress notes, etc.) - Assist our members with maintaining benefits such as Social Security, Supplemental Security Income, Medicaid and Medicare coverage, and Food Stamps. - Assist members to resolve problems in living such as housing, utilities, the judicial system, and general safety. - Report abuse or neglect immediately when observed or reported. - A comprehensive job description may be provided during the interview process
