Overview

The Regional Registered Nurse ("RRN") is a critical player in coordination of health care services for Care Design NY (CDNY) Care Coordination Organization/Health Home (CCO/HH) members.


This team-based approach to care and provide health services to individuals with intellectual/ developmental disabilities (I/DD), through active partnerships with individuals, their caregivers/families, community resources, and their physicians. Facilitates a strong, stable person-centered approach to holistic service planning within and across settings to achieve coordinated, high-quality care.


The RRN is a critical part of the team supporting the members and families we support with access to, and ongoing support related to all health care supports and services. The RRN is a source of expertise related to all health care support and services and works collaboratively with all team members to ensure a holistic approach to care. Including CMS Medicare-Medicaid Plan, NYSDOH CCO, OPWDD Reporting Requirements, and other measures to assess CDNY performance and to improve the quality outcomes for the people supported by CDNY.

Responsibilities

  • Facilitate access to primary care, specialty care, and other healthcare resources and services.
  • Provide health education and support to members, families, caregivers, and staff.
  • Serve as a clinical resource to care managers and interdisciplinary teams for complex healthcare needs.
  • Participate in case reviews, Clinical Rounds, and care coordination activities to improve member outcomes.
  • Support care transitions, including emergency room visits, hospitalizations, discharge planning, and follow-up care.
  • Collaborate with providers and care teams to address unmet healthcare needs and improve quality of care.
  • Ensure compliance with applicable CMS, NYS DOH Health Home, and OPWDD requirements.

Qualifications

  • A Registered Nurse licensed to practice in New York state required
  • BSN is preferred
  • 2+ years of experience in health services preferred
  • 2+ years of experience collaborating with people with IDD preferred
  • 3+ years of work experience in a managed care plan or health home is preferred
  • Experience working collaboratively with various departments in fast-paced environment
  • Excellent organizational, interpersonal, and verbal and written communication skills required; ability to attend to detail, organize tasks/priorities and complete with accuracy
  • Exemplary written and oral communication skills
  • Proficiency in navigating through electronic record software programs
