Overview

The Care Manager Intern supports CDNY Care Management services in accordance with the model of care and
regulatory requirements of the applicable program or line of business.


The Care Manager Intern provides support to care management teams by completing non-billable tasks and duties
assigned by their designated regionalsupervisor. The Care Manager Intern performs hands-on, member-facing functions
that assist Care Managers in coordinating and implementing the person-centered planning process.
This is a learning-focused internship designed to provide the relevant experience necessary to gain the competencies
and qualifications to become a Care Manager. This Care Manager Intern is a time-limited position that can last up to a
maximum of 12 months.

Responsibilities

• Responsible for assigned tasks that assist a group of Care Managers in supporting members and families.
Tasks may include, but are not limited to, the following:
o Assisting in obtaining documents needed for person-centered planning and core Care Management
responsibilities (i.e., medical records, Staff Action plans, documents required for waiver applications)
o Contacting providers to assist Care Managers with service linkages/referrals (i.e., calling providers
for waiver service openings; helping facilitate the completion of referral packets)
o Attend regional rounds consistently: Present at regional rounds to assist Care Management with
the most complex members.
o Assisting Regional Care Management Teams in achieving performance metrics through regional
tracking systems.
o Assist with coordinating scheduling meetings with the assigned Care Manager, Member/Family, and
members of the IDT.
o Assist in gathering necessary documentsthat ensure a complete and comprehensive record. Engage
with members/family, service providers, and members of the IDT to obtain and upload documents
into the member’s record for the assigned Care Manager’s review and action.
o Participate in the CAS assessment with the assigned Care Manager as requested.
o Attend the Care Manager review of CAS assessments as requested.
o Attend LP meetings with the Care Manager.
o Review documentation and upload it to preventative care with the director’s oversight.
o Participate in the completion of DDP2s with the assigned Care Manager.
o Prepare biopsychosocial as needed.

 

Additional requirements:
• Ensure that all assigned duties are completed on time.
• Ensure confidentiality and adhere to HIPAA guidelines.
• Navigate Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
• Report abuse or neglect immediately when observed or reported.
• Meet all training requirements on time.
• Other duties as assigned.

Qualifications

  • High School Diploma
  • A completed or in-process bachelor’s degree in human services is preferred
  • Personal or professional experience working with individuals with I/DD preferred

Working Conditions & Physical Requirements

This position requires field or office duties, primarily from their home office, and requires them to work from
locations within 90 minutes of their assigned fieldwork or CDNY or PS office.
This role routinely uses standard office equipmentsuch as laptops and smartphones.
While performing the duties of this job, the employee is occasionally required to stand, walk, sit, use hands
to finger, handle, touch objects, tools, or controls, and reach with hands and arms.
Must have adequate visual acuity to read correspondence, computer screen, and forms; manual dexterity
to operate keyboard; speaking and hearing enough to communicate by phone or in person at average
volumes.
May be required to adjust hours, days, and work schedules as needed to support the organization's overall
needs.
Must convey detailed, importantspoken or written instructionsto others accurately.
Must have the ability to receive and comprehend detailed critical information through oral and/or written
communication.
Ability to lift and carry materials up to twenty pounds.
The noise level in the work environment is usually low.
